I advise using the faster weapon in mainhand (left) - it seems the IAS from off-hand might not even be part of the equation. Your off-weapon IAS and mainhand IAS are definitely part of it. 50 IAS (poorly rolled Grief + Andariel's Visage) = 4 stacks of Frenzy to reach max speed. 68 (35% andy + 33% Grief) at 2 stacks.

The additional damage from the Holy Fire will not increase the damage on the fireball it will add to the sword attack damage.
